Does Moda Health Cover Acupuncture?

Good news for Moda members in Portland: starting October 15, Alexis Lively Acupuncture is in network with Moda Health.

What does that mean for you?

If your Moda plan includes acupuncture, I bill Moda directly. You pay only what your plan says you owe, such as a copay or coinsurance.

You will need a referral

Moda requires a referral for acupuncture. Ask your primary care provider for a referral to Alexis Lively, L.Ac., before your first visit using insurance.

How to check your coverage

Plans differ, so a quick call before your first visit helps:

  1. Call the member services number on the back of your Moda card.

  2. Ask whether your plan covers acupuncture and how many visits per year it allows.

  3. Ask what your copay or coinsurance is, and whether you have a deductible to meet first.

How to book

Book online and choose a "Using Insurance" option. If you are a new patient, email a photo of the front and back of your insurance card to hello@alexislively.com before your visit.

What a visit includes

Each visit is 60 minutes: 45 minutes of acupuncture and 15 minutes of cupping, with infrared heat. New patient visits start with a full health history and evaluation.

Other insurance

I am working on becoming in network with BlueCross BlueShield and UnitedHealthcare. In the meantime, I can provide a superbill you can submit to your insurance yourself.
