Pricing and Insurance

We are currently accepting

  • Motor Vehicle Accidents and Workers Comp.

  • Moda Health (in network starting October 15, 2026)

Coming soon

We are working on becoming in network with these plans and will update this page once it is official:

  • Regence/Anthem/BlueCross BlueShield

  • United Healthcare

If we are not in-network with your insurance policy, we can supply you with a superbill for potential reimbursement from your insurance company. We also accept HSA/FSA.

Unfortunately, at the time, we are not in-network with any OHP, Medicare or Medicare advantage plans.

Once you schedule your initial visit, we will check your benefits for you, however it is the patient's responsibility to know and keep track of their own acupuncture benefits.

If we will be billing your insurance, please send a copy of the front and back of your insurance card along with your address and date of birth to hello@alexislively.com

Please give us 24-48 hours to verify your benefits. Thank you.

What is a superbill?

A superbill is a detailed receipt for your visit. It lists my license information, the date of your visit, and the codes your insurance company needs to process a claim. You pay for your visit when you come in, and I give you a superbill afterward so you can ask your insurance to pay you back.

How to get reimbursed with out-of-network benefits

Out-of-network means I am not contracted with your insurance plan. Many plans will still pay you back for part of the cost. Here is how it works:

Step 1: Call your insurance before your first visit. Ask if you have out-of-network benefits for acupuncture, how much of your deductible is left, what percentage they pay back, and whether there is a limit on visits.

Step 2: Pay for your visit at the time of service. HSA and FSA cards work too.

Step 3: Ask for your superbill. I will give it to you after your visit.

Step 4: Send the superbill to your insurance. Most plans let you upload it on their website or app, or you can mail it in with their claim form.

Step 5: Your insurance pays you directly, usually by check or deposit. This often takes a few weeks.

Good to know: if you have not met your deductible yet, your plan may not pay you back right away, but those visits still count toward it. Every plan is different, so a quick call to your insurance is the best way to know what to expect.

Pricing

What is a time of service discount?

This is not our policy choice. Because we bill insurance, insurance billing laws and our insurance contracts require us to charge every patient the same standard fee. By law, we cannot have one price for cash patients and a different price for insurance patients.

What the law does allow is a time of service (TOS) discount. If you are not using insurance and you pay in full on the day of your visit, you get a set discount off the standard fee. The discount must be the same for everyone who qualifies, and the rules limit how large it can be, so the time of service rates below are the lowest prices we are allowed to offer.

Prices if you are not using insurance:

  • New patient visit (evaluation, then 45 minutes of acupuncture and 15 minutes of cupping, plus infrared): $300

  • Returning patient visit (60 minutes: 45 minutes of acupuncture and 15 minutes of cupping, plus infrared): $180

  • Low Barrier To Acupuncture (returning patients): $120. An acupuncture-only session: a full appointment with acupuncture as the treatment, no cupping or add-ons. Self-pay only, and available only until I am in network with BlueCross BlueShield and UnitedHealthcare.

  • Facial Rejuvenation Acupuncture: $250

The time of service rate is for patients not using insurance who pay in full on the day of the visit. Low Barrier To Acupuncture and Facial Rejuvenation Acupuncture are set prices.

Cancellation Policy

Your appointment time is reserved just for you. A late cancellation or missed appointment leaves a hole in the acupuncturists day that could have been filled by another patient. As such, we require 48 hours notice for any cancellations or changes to your appointment. Patients who provide less than 48 hours notice, or miss their appointment, will be automatically charged a cancellation fee of the cost of their treatment as we are unable to bill insurance for missed appointments.

Patients that are more than 15 minutes late are considered a no show and will be charged a cancellation fee.
